David Leon Choulat, age 57, from Columbus, passed away Tuesday, July 14, 2009. David was born in Columbus on August 2, 1951 to Leo B. and Lois Choulat. He was preceded in death by his father, Leo B.; grandson, Dylan; and nephew, Craig Pelphrey. Survivors include his wife of 36 years, Donna Choulat; children, Angela (David) Choulat, David (Krystal) Choulat II, and Aliviah (Rich) Sauers; grandchildren, Elise, Chasity, Logan, Collin, and Matthew; siblings, Debra (Raymond) Brown, Mark Choulat, Marsha (William) Siders, Mary (David) Wilson, Timothy Choulat, and Eleisha Choulat (Randy Isom); special thanks to Eleisha Choulat for giving her brother David the gift of life through organ donation. Family and friends may call on Friday, July 17, 2009 from 4:00 PM until time of funeral service at 6:00 PM at Newcomer Funeral Home, SW Chapel, 3393 Broadway, Grove City, Ohio. Donations in David's honor may be made to the National Kidney Foundation 30 East 33rd Street New York, NY 10016.
